Health Care Proxy

A health care proxy, also called a medical power of attorney, is a legal document in which you designate a person, called an agent or health care agent, to make medical decisions on your behalf. You should also prepare and sign a living will which is a separate document that specifies your health care wishes. See the sample forms on this website in the estate planning forms section.
